- No appointment needed to visit Josselyn for the first time
- Please allow 2 hours or longer for a visit

What To Bring
- Identification to complete intake process, including
- Valid identification
- Proof of health insurance (if applicable)
- Proof of income (if uninsured): This helps us determine your eligibility for a sliding scale fee under our Affordability Fee Program. Acceptable documents include a W-2, your two most recent pay stubs, or your most recent Form 1040 tax return.
- If you are unable to provide documentation, you may self-declare your income.
